2. FACTS - Tuition Payment Application

Complete the FACTS online form. Please contact the school office if you need a hard copy. 

Note:  Beginning June 1, 2015, Visa will be added as an accepted payment option.  Families will be able to use Visa to enroll in a family payment plan.

3.  FACTS - Grants and Aid Application

Complete and submit the Grant & Aid Assessment form.  It may be started on-line and sent electronically, but please remember that other documentation my be required for full consideration of available scholarships, grants and aid.

FACTS Payment History Report

 

Parents may need to access this report for tax purposes or for personal records.  In order to help, FACTS has enhanced our system to allow families to print their payment history via calendar year. The payments includes system processed payments and manually applied payments and provides a calendar year or date range summary of payments made broken down by term.

4. African Ancestry Tuition Assistance Award

The funding for this assistance comes from the generosity of people from across the Diocese and across the country who participate in the Black and Indian Missions Collection.  Funds are available for qualified applicants in grades K-12 attending Catholic schools in the Diocese of Raleigh.

Application Instructions -

Download and complete the scholarship form to apply for merit aid and submit to the office of Our Lady of Perpetual Help Catholic Church, 331 Hammond St., Rocky Mount, NC  27803.

Deadline for submission is February 27, 2017.  Recipients will be announced on April 2, 2017.

 

Purpose:  The fund is established to provide merit award assistance in the amount of $1000.00 to worthy applicants in Kindergarten through Eighth Grade who attend or will attend Our Lady of Perpetual Help Catholic School.

 

Criteria:

 

  •  Recipient's family must be a registered parishioner of Our Lady of Perpetual Help Catholic Church.

 

  • Family must have demonstrated involvement in the parish by their giving of time, talents, and treasures to the parish and community.

 

  • Prospective students should demonstrate an eagerness to learn, have strong study skills, and take responsibility for their education by completing homework and projects in a timely manner and to the the best of their abilities.  Students not currently enrolled at OLPH Catholic School should include documentation which supports their scholastic qualifications.

 

  • Recipients will accept a ministry or projects selected by the scholarship committee that help with their formation and be a relection of the character of Brianna and her good works.

 

  • Child to write a letter on why they want this merit award.
